Community Health Connections (CHC) is a nonprofit, Federally Qualified Health Center (FQHC) with five locations in North Central Massachusetts offering walk in urgent care, primary family medical and pediatric care, preventative and restorative dental care, oral surgery, behavioral and mental health services for children and adults, substance use disorder treatment, specialty services including optometry eye care, optical shop, acupuncture, nutrition consultations and podiatry.

The Corporation assisted the Northeast Behavioral Health Care Consorlum, a 501(c) 3 organization, with the development, implementation and management of the HealthChoices managed behavioral health program in the NBHCC Service Area, serving approximately 82,000 Medicaid beneficiaries enrolled in this program in the Counties of Lackawanna, Luzerne, Susquehanna and Wyoming, administering the NBHCC contract with the PA DPW.
