(THE ORGANIZATION) IS A NEW YORK NOT-FOR-PROFIT ORGANIZATION WHOSE MISSION AND PRINCIPAL ACTIVITIES ARE TO PROVIDE SERVICES FOR THE DEVELOPMENTALLY DISABLED OF NEW YORK THROUGH THE OPERATION OF RESIDENTIAL ALTERNATIVE PROGRAMS, AN INTERMEDIATE CARE FACILITY, AND OTHER PROGRAMS CERTIFIED BY THE NEW YORK STATE OFFICE OF PEOPLE WITH DEVELOPMENTAL DISABILITIES (OPWDD).
