PACE OF GUILFORD & ROCKINGHAM COUNTIES INC
PACE of the Triad provides comprehensive services for medical fragile older adult who desire to remain in their community setting. Although medically eligible for nursing home care; PACE participants are able to live at home or with family and friends as a result of family and friends and the PACE program. PACE services include but are not limited to primary care, rehabilitation therapies, activities, recreational therapy, social services, meals, nursing, transportation, personal care, medications, acute care, medical specialist and homecare. While providing services for the frail older adult, PACE of the Triad also supports caregivers who are providing the care after PACE hours through education, training, respite and support.
