The Living at Home/Block Nurse Program dba Living at Home Network is a neighborhood-based system of service delivery for older persons which uses neighborhood professional and volunteer personnel to provide, directly or in coordination with appropriate agencies and resources, needed health and social services that enables them to remain at home.
The purpose of Living At Home/Block Nurse Program, Inc. dba Living at Home Network is as follows:
1. To provide information about the Living at Home/Block Nurse Program (LAH/BNP) to interested communities.
2. To provide technical support to communities in implementing the model.
3. To maintain and monitor replicating programs
