Care Coordination services are provided by a specialized workforce – Community Health Workers. Through advocating, educating, connecting and supporting, CHW’s our partners with clients and the clinical and social services received in the community. Our mission: To build a primary care integrated community-centered system for the uninsured and other vulnerable populations in our region.
Our vision: To find consumers through community outreach and working with targeted physicians, enroll consumers, connect them to care and resolve barriers to care.
