Community Health Connection provides quality and accessible health care across all life cycles on a nondiscriminatory basis regardless of ability to pay, that respects the dignity and culture of our community now and into the future. We accept patients regardless of insurance status and we offer a sliding fee scale discount to patients below 200% of poverty guidelines and enable patients access to lower cost medications.
