The Wellness Plan Medical Centers deliver affordable and accessible quality medical services to our THE PRIMARY PURPOSE IS TO RECEIVE AND ADMINISTER FUNDS IN SUPPORT OF THE OPERATION OF FEDERALLY QUALIFIED HEALTH CENTERS TO ENABLE THEM TO PROVIDE HEALTHCARE SERVICES TO UNDERINSURED AND UNINSURED INDIVIDUALS; TO ENGAGE IN SUCH OTHER CHARITABLE FUNCTIONS TO ENHANCE THE QUALITY OF LIFE OF CITIZENS OF SOUTHEASTERN MICHIGAN.

PatientCentered Medical Home The Wellness Plan Medical Centers is a patientcentered medical home PCMH a model of care where patients engage in a oneonone relationship with their primary care provider who acts as a liaison to further care from an extensive medical team. Community Involvement The Wellness Plan Medical Centers community involvement team acts as a connector for patients and the community to build relationships and provide support.
