History

The concept for Grace Medical Home was inspired by a medical mission trip to the Dominican Republic, where local physicians helped transform a children's clinic into a comprehensive medical home. Recognizing similar needs in Orlando, these physicians along with other healthcare professionals, business leaders, and volunteers collaborated to establish Grace Medical Home. The organization received its 501(c)(3) status in 2008 and began operations in 2010.

Mission and Services

Grace Medical Home operates as a medical home, an approach focused on providing comprehensive primary care that ensures continuous medical attention and coordinated care for patients. It is not limited to a physical building but serves as a hub where patients can access well and sick visits, chronic disease management, and a wide range of over 35 medical specialties. Since its inception, Grace Medical Home has served more than 5,000 patients, significantly reducing emergency room visits by more than $2.4 million since 2015. The organization relies on over 400 physician volunteers and has been recognized for its community impact, including being named a 2022 Bank of America Neighborhood Builder.
