History

Sowing Seeds Health Inc originated from Sowing Seeds Music Ministry, founded by Dr. Shawn Hamilton and a group of Christian friends in 2009. Initially, the ministry focused on sharing healing through music in various settings. However, during the pandemic, the organization shifted its focus towards community health, particularly highlighting the need for community health workers (CHWs) with lived experience and cultural diversity. In 2022, the nonprofit was rebranded as Sowing Seeds Health Inc., establishing Sowing Seeds Community Health Center to expand its scope to include spiritual, physical, and mental health services.

Services

The organization provides several key services:

• Enhanced Care Management (ECM): This program is designed to offer intensive care management for individuals in need.
• Community Health Worker (CHW) Services: Sowing Seeds trains and certifies CHWs, equipping them with the skills to support medically underserved populations.
• Community Support Services: These include services like asthma remediation and environmental accessibility adoption.
• CHW Instructor Training and Certification: The organization offers training and continuous skill development for CHW instructors.

In the 2023-2024 period, Sowing Seeds received substantial funding, including an IPP grant and an award from CITED funds, totaling $3 million. This support is aimed at building the organization's infrastructure and capacity to deliver CHW and ECM services. By 2025, Sowing Seeds plans to serve 35 counties across California.
