WAS FORMED TO PROVIDE PATIENT NAVIGATION TRAINING PROGRAMS WHICH ASSIST INDIVIDUALS IN OBTAINING TIMELY SCREENING, DIAGNOSIS AND TREATMENT OF CANCER AND OTHER CHRONIC DISEASES; TO DEVELOP OR EXPAND PATIENT NAVIGATION PROGRAMS TO BENEFIT PATIENTS WHO MEET BARRIERS TO TIMELY DIAGNOSIS AND TREATMENT OF CANCER AND OTHER CHRONIC DISEASES; TO EXPAND THE AWARENESS OF CARE GIVERS OF THE NEEDS OF PATIENTS UNDERGOING DIAGNOSIS AND TREATMENT OF CANCER AND OTHER CHRONIC DISEASES; AND TO ASSIST THOSE LIVING WITH CANCER, AND OTHER CHRONIC DISEASES, IN OBTAINING AVAILABLE SUPPORT SERVICES.
