Patients admitted to one of our nine facilities have often had a recent hospital stay and experienced a decline in functional ability. Our team of talented nurses and therapists, along with other skilled health professionals, work directly with each patient to develop an individualized care plan to meet each patient's goals. As a result, many of our patients are able to return to their own home after completing a successful rehabilitation program. Our Post Acute centers offer only post acute care and focus only on outstanding clinical outcomes and successful discharges to the home setting.
