Reliance Community Care Partners – Your Community Care Advocate WELCOME TORELIANCE COMMUNITY CARE PARTNERS. The name Reliance represents the role we play in connecting those we serve with the care they need. A communitybased and personcentered organization Reliance provides and coordinates supportive services to help disabled and older adults live as meaningfully and independently as possible. Weve served the West Michigan community since 1979 and will continue to be a trusted advocate and knowledgeable resource for lifemanaging care.
