Total Community Care provides health care and supportive services that help seniors and frail individuals live with dignity in their homes and community. An expert, multidisciplinary team coordinates health care, home care and support for family caregivers. Participants benefit from recreation, activities, interaction with friends and clinical services at adult day centers. Personal care, skilled nursing, light chores and homemaker services assist participants at home.
