THE COUNCIL OF AGING PROVIDES AGING ADULTS AND THEIR SUPPORT SYSTEM WITH SERVICES AND OPPORTUNITIES THAT ENABLE THEM TO ACHIEVE AN OPTIMAL, SELF DEFINED, QUALITY OF LIFE. THIS IS ACHIEVED THROUGH THE PROVISION OF EDUCATION, NUTRITION SERVICES, CASE MANAGEMENT SERVICES, IN-HOME ASSISTANCE AND COMMUNITY OUTREACH. THE LAKE COUNTY COUNCIL ON AGING PROVIDES SUPPORT SERVICES, PROGRAMS AND ADVOCACY TO HELP LAKE COUNTY SENIORS LIVE DIGNIFIED AND INDEPENDENT LIVES. THE AGENCYS PRIMARY RESPONSIBILITIES ARE TO DESIGN, PROMOTE AND IMPLEMENT SERVICES TO ADDRESS THE IDENTIFIED NEEDS OF THE COMMUNITYS ELDER POPULATION AND TO COORDINATE EXISTING SERVICES IN PARTNERSHIP WITH PUBLIC AND PRIVATE ORGANIZATIONS. THE AGENCY PROVIDES INFORMATION, ASSISTANCE AND DIRECT SUPPORT THAT ALLOW SENIORS TO MAINTAIN THEIR INDEPENDENCE AND SUSTAIN THEIR QUALITY OF LIFE.
