Non profit home health care and hospice for all ages, all incomes, in Franklin County VT. We provide medically necessary care, regardless of ability to pay. We provide high quality health care programs and related support services in a community setting to residents of all ages of Franklin County, VT. We also provide Nurse Family Partnership services to low income mothers and babies in Franklin, Lamoille and Grand Isle counties in Vermont. We are committed to excellence in service. We strive to meet the needs of our clients in a professional manner that promotes health, independence, comfort, dignity and quality of life.  We fulfill our mission by: promoting and maintaining quality health care promoting wellness aiding in the prevention of diseases providing comprehensive health care within the home setting collaborating with other groups in developing and implementing community health care and service programs contributing to the knowledge which will improve community health and service practicesWe are also committed to providing medically necessary home health care to everyone who needs it regardless of their ability to pay. Each year, we provide over $500,000 in charity and uncompensated care, and seek to fill this gap through our fundraising efforts.We recruit and support professional home health care experts to provide the highest quality care to our community. We provide them with leadership and educational opportunities to ensure they are abreast of all the latest innovations in care and to build leaders for our future.Our programs and services cover a full range of home health and support services for children and adults including: rehabilitation therapies for all ages, maternal child health and lactation consultation, support of at-risk mothers and babies through Nurse-Family Partnership, skilled nursing, hospice and palliative care, homemaker support, high-tech nursing, long term care, private duty care, and wellness programs and screenings.The best testament of the value of our care to the people in our community is their overwhelming support of our work through patient satisfaction scores and the trust they put in our agency and our staff each day.
