When others said no, we said not giving up. 💙 At UDS Foundation’s Advanced Care Management Solutions (ACMS) program, we believe everyone deserves a path forward—no matter how complex the case. After years in the hospital and hundreds of rejections from care facilities due to serious health and behavioral challenges, one of our participants was running out of options. That’s when our ACMS Care Manager stepped in—with relentless advocacy and compassion. They built trust through repeated hospital visits, bringing their favorite treats - sushi and hot wings, coordinated with state agencies, explored facilities across the region, and advocated tirelessly to get him the services and placement he deserved. Thanks to their dedication, he was finally accepted into a facility and discharged after three long years. Just three days later, they met with the participant at the nursing home, delivering clothing and personally checking on his adjustment. They connected with the facility's Social Worker and Speech Therapist, ensuring a seamless transition.
