Day Service Services are based on the needs of the individual being served Provides skills to enhance socialization skills Enables individuals to have opportunity for participation in community inclusion Offers behavioral support as well as coping skills Training and assistance in daily life skills Development of vocational skills Residential Services Assist the individuals to successfully live independently in the community Provide support in activities of daily living Support the desires needs and goals of each individual served Supports and services are provided in accordance with needs identified in the Person Centered Support Plan PCSP Targeted Case Management Assist the individual with identifying selecting coordinating and obtaining natural or paid supports Oversees all programs including but not limited to HCBS services employment and self direction of services Information andor assistance with funding and record keeping for continuation or beginning of HCBS services Assess individuals needslife goals and generate a Person Centered Support Plan PCSP Gather and complete information needed to ensure the retention of benefits and services Day Service Residential Service Targeted Case Management . .
