Building a new vision for community health and primary care in underserved communities of the Dominican Republic Our mission is to improve community health in the Dominican Republic by building local capacity for quality primary care and public health. Our work revolves around empowering community health leaders, partnering with and strengthening the primary health care system, and promoting public health and patient access to quality prevention, treatment, and management services. We develop on-the-ground programs in four "core" partner communities to understand what works, and scale our best solutions through strategic partnerships with primary care clinic networks and nationally-driven initiatives. We are the only organization in the country that focuses on fighting chronic disease, by combining a Community Health Workers Program with direct provision of primary health care.

Through empowering local populations and strengthening primary care we build sustainable communitycentered health systems that address the root causes of health disparities such as poverty and limited access to essential services. Our mission is to create a healthier more equitable world for all. We are excited to share the success of our recent Medical Service Trip during which our team delivered vital healthcare services to 175 individuals across more than 20 communities reaching those who often face significant barriers to accessing care.
