IS AN AGING SERVICES ACCESS POINT AND DESIGNATED AREA AGENCY ON AGING THAT PROVIDES SERVICES WHICH INCLUDE IN-HOME SUPPORT, INFORMATIONAL SERVICES, NURSING HOME ADVOCACY, LEGAL ASSISTANCE, MEAL SITE TRANSPORTATION AND HEALTH PROGRAMS FOR ELDERS AND OTHERS.

Our Programs Services At Access Care Partners our mission is to preserve the dignity independence and quality of life of older adults and persons with disabilities and chronic conditions OF ALL AGES. We offer a wide range of services to support both caregivers and people receiving care including Personal Care Management We assist individuals with chronic conditions or disabilities by helping them hire and supervise Personal Care Attendants. Nutrition Services We offer healthy wellbalanced meals prepared locally and delivered to your home or served in community settings.
