Carl Stotz initiated Little League after agreeing to play catch with his nephews, who were too young for traditional baseball leagues. In 1974, the organization began admitting girls and also introduced softball programs for both juniors and seniors. The Challenger Division was established in 1990 to include children with physical and mental disabilities.

- Local League: Each local league serves its community by providing baseball and softball programs. These leagues are autonomous but must adhere to Little League International's rules and policies.
- District: Typically comprising 10 to 20 local leagues, districts are managed by a volunteer District Administrator, who supports and oversees league operations.
- Region and International Headquarters: Little League International, based in Williamsport, Pennsylvania, oversees regional centers and provides resources to support local programs. The organization is divided into five regions in the United States and several international regions.
