WELC's mission is to offer preparedness, guidance, and information so Westchester families can live more confidently with serious illness and at the end of life. WELC envisions a community where all individuals with serious illness or at the end of life, and their families, are prepared for all stages of the health journey • promote the completion of advance care planning documents and value-based conversations about healthcare wishes and goals • provides directions to navigating the healthcare system and accessing services and resources • expands awareness of issues surrounding the end of life • functions as a clearing-house for information and a network among lay and professional care providers; • identifies gaps in services and promotes the creation of programs to fill those gaps.
