Whittier Street Health Center's mission is to provide high quality, reliable and accessible health care and support services to promote wellness and eliminate health and social disparities. To accomplish this mission, we have developed an innovative population health management program -- the Boston Health Equity Program (BHEP). This evidence-based model of care combines primary, specialty and urgent care with social supports, care management, patient navigation, and wellness services. BHEP goals are based on CMS Triple Aims to: improve population health by coordinating and integrating health care and community-based services; improve care by increasing access to services; and reduce costs by avoiding unnecessary emergency room and hospital visits.

Whittier is licensed by the Massachusetts Department of Public Health DPH and is a FTCA Deemed Facility.
