Adult Services Child Family Services Designated Lead Health Home In New York State Circare is designated by the New York State Department of Health as a Lead Health Home. In collaboration with Health Homes of Upstate New York HHUNY we manage a network of providers in 10 Central New York counties who work with individuals with complex chronic health andor behavioral health needs to help them achieve positive health outcomes.
