Community Health Access Network Community Health Access Network Making Health Care ServicesAccessible With Our Network Community Health Access Network CHAN is the only Health Center Controlled Network HCCN in New Hampshire. Established in 1995 CHAN provides health care IT including the electronic health record as well as trainingtechnical support for 15 Federally Qualified Health Centers FQHC including 3 Healthcare for the Homeless Programs. CHANs focus is to automate the primary care health record. CHANs systems include a fully integrated Electronic Medical Record EMR which supports over 130 providers and partner hospitals.
