In 2003, the county began to supplement the volunteer efforts by contracting with Carilion Transport Service. However, the rising costs associated with this arrangement led to the decision to create a county-operated service. Consequently, FCEMS was formed in 2004 with a focus on hiring county employees to deliver reliable emergency medical care in the region.
Operations
FCEMS is responsible for the entirety of Floyd County, ensuring rapid response to emergencies throughout the area. The organization maintains a fleet of ambulances, aiming to keep at least two operational at all times, staffed by highly trained emergency medical service providers.
The service functions under a model where billing for emergency medical services is conducted through private insurance companies, Medicare, and Medicaid, ensuring the sustainability of its operations.

In addition to providing emergency medical care, FCEMS is committed to community involvement and education. The organization hosts students from various Emergency Medical Technician (EMT) and Paramedic programs, offering practical experience in rural emergency medical services.
