ANN'S HOME IS A 376 BED LICENSED SKILLED NURSING FACILITY. SPECIALIZED CARE OFFERED IN THIS FACILITY INCLUDES 34 UNITS DEDICATED TO PROVIDING SHORT-TERM POST-ACUTE REHABILITATION AND COMPLEX MEDICAL SERVICES, AND 36 UNITS PROVIDING DEMENTIA CARE. ST. ANN'S HOME ALSO OPERATES HOME CONNECTION, A 30 SLOT MEDICAL ADULT DAY PROGRAM, AND ADULT DAY PROGRAM AT DURAND, A 36 SLOT MEDICAL ADULT DAY PROGRAM.
