Requirements 18 years of age or older Have a diagnosis of Down SyndromeTrisomy 21 Have attended at least one medical or health related appointment within the last 6 months Have held some autonomous responsibilities such as a job or volunteer role This study consists of a 5 to 10minute long anonymous eligibility survey and a followup 30 to 60minute survey if patients meet the eligibility criteria and complete a consent or assent process.
