Who We Are HomePlus Care is a 501 c 3 taxexempt nonprofit organization and Home and Community Based Services provider. Validated by the Office of Long Term Living of PA Department of Human Services we serve individuals living in Philadelphia Bucks DelawareMontgomery and Chester counties. We provide Personal Assistance Services as coordinated by Managed Care Organizations MCOs who operate health careservices under contracts with the PA Department of Health and the PA Department of Human Services via Community HealthChoices Long Term Services and Supports of Medicaid WaiversManaged Care Waivers and Home and Community Based Services Waivers.
