Meadowood Health Partnership Offices Located At Meadowridge Library 5902 Raymond Road Ste. 160 Madison WI 53711 Who We Are Sheray Wallace Founder and Director After years as a certified nursing assistant and community organizer I founded MHP in 2016 to provide wraparound health support to my neighbors. I run MHP on a parttime basis with support from a few dedicated volunteers. The COVID19 pandemic unveiled a growing need to expand MHPs services both geographically and strategically.
