Everett Transitional Care Services Everett Transitional Care Services Helping Transition to the Next Level of Care Bethany of the Northwest Regional Medical Center and Swedish Edmonds Hospital are working together to support the needs of the community. This joint partnership Everett Transitional Care Services serves a very important purpose for this in Snohomish county in need. Initially we collaborated on a special needs rehab unit. In 2020 as the community needs changed the focus of our partnership turned to those who are struggling to find placement in skilled nursing assisted living adult family homes or other community locations.
