WHAT WE OFFER At the heart of our mission is a commitment to fostering a resilient and thriving community through a spectrum of vital services. Our Community Health Workers are certified through training with DHSS and provide a range of services to address the social determinants of health. OUR FOCUS Education We provide a community information feedback loop through doortodoor canvassing a Route 9 newsletter and tabling at community events.
