Inland Empire Health Information Organization We Link People to Data IEHIOs purpose is to connect people with data by coordinating data sharing services not only to improve treatment at the point of care but also to address the psychosocial determinants of health to support wholeperson care efforts that improve the health of all residents in the Inland Empire. IEHIO is a nonprofit organization supporting interoperability for healthcare and the community. IEHIO Projects Empower Care Social Drivers of Health Initiative IEHIOs Empower Care Initiative funded by Physicians Foundation supports a limited amount of solo and smallgroup medical offices in Riverside County in integrating Social Drivers of Health SDOH screening and resource navigation into patient care. Running from 20252026 the initiative helps providers identify social needs connect patients to community resources and enhance wholeperson care through streamlined screening and referrals.
