Community Health Partnership | Collaboration in Action | Colorado Springs CHP provides planning administrative and data services for the Pikes Peak Continuum of Care to build an effective system of care for people experiencing homelessness. CHP works with community members local health care systems and organizations to ensure that LGBTQ people and their families have reliable access to safe competent and affirming mental and physical health care. CHP provides support for a coalition of stakeholders engaged in communitycentered planning to transform the way the Pikes Peak region addresses behavioral health care needs. CHP serves as the backbone organization for a countywide collaborative aimed at reducing suicides by 20 by the year 2024.
