Home - Community Health Center Network k A healthy community begins here. Starting on January 1 2024 our MediCal patients in Alameda County will be assigned to a single health insurance plan through Alameda Alliance for Health. Please note that if your healthcare membership is going to be impacted by this shift you should have a letter by mail from your current health insurance plan with all the details and next steps. If you have any questions or concerns please contact your health plan.
