Community Care Networks (CCN) is a 501(c)(3) nonprofit charitable organization whose mission is dedicated to creating opportunities for individuals with disabilities. CCN assists people, including veterans, with mental and physical disabilities, by providing life-enhancing opportunities. Active involvement in the community is a priority, but those opportunities for disabled individuals with low incomes can be difficult to obtain. CCN will strive to provide meaningful experiences for our members. CCN's core value is CARING, and we are dedicated to caring for and changing lives, one person at a time.
