We work toward the prevention of homelessness and creation of lasting solutions for homeless and at-risk families and individuals throughout Colorado. Creating Lasting Solutions to homelessness in Colorado for over 30 years. The Mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for families, children, and individuals who are experiencing or at-risk of homelessness throughout Colorado. CCH advocates for and provides a continuum of housing and a variety of services to improve the health, well-being and stability of those it serves. Since its founding 40 years ago, the organization has earned state and national recognition for its integrated healthcare, housing and service programs. The Coalitions comprehensive approach addresses the causes of homelessness, as well as the consequences, offering critical assistance to over 20,000 individuals and families each year.

Two years in and Denver’s Housing to Health (H2H) program continues to prove that housing IS health care. ✅ 70,676 days of stable housing ✅ 119 people stably housed, with 105 remaining housed for a full year ✅ $1. 36M in success payments made based on real results ✅ Fewer ER visits and jail stays, thanks to housing and health care partnerships H2H is a first-of-its-kind Pay for Success initiative that links permanent supportive housing with health and mental health services for people experiencing chronic homelessness who frequently interact with emergency health care services and the criminal legal system.
