The mission of Community Health Alliance of Pasadena (ChapCare) is to provide excellent, comprehensive, and innovative healthcare that is accessible to residents of the San Gabriel Valley. Since its inception, community members have played a significant role in the planning and implementation of ChapCare services. ChapCare's area of expertise is its ability to provide high-quality primary healthcare services. Through our team-based primary healthcare model we are able to manage our patients' overall health and well-being.

AltaMed is the largest independent federally qualified health center FQHC in the nation serving more than 500000 patients in Los Angeles and Orange counties. We have expanded our mission to include the San Gabriel Valley.
