With a 20year history in care coordination across medical behavioral health and social service systems CareReach has a demonstrated track record of effective program management and is a respected fiscal agent for the management of grants and contracts. We are dedicated to serving our community by engaging in programs that support community health initiatives through implementation and direct staffing resources. We work with individuals to help them reach their health goals and we collaborate with local partners to make clinical and community services easier for people to reach.
