Community Heart About Community Heart is a communitybased nonprofit organization with a mission to connect those affected by cancer in the Tracy community with a network of support. When someone in our community receives a cancer diagnosis patients and their families do not always know who or where to turn to for information and support. At the same time there are many community members including cancer survivors and those who have seen a loved one friend or acquaintance battle cancer who want to reach out but may not know how. The question then arose how do we bridge the gap between those who are seeking support and those who would like to provide support.
