Our community partners request our assistance with community health and needs assessments, health improvement plans, strategic planning, and program monitoring. We are part of a network of 11 planning agencies that serve all 67 counties in Florida. Health data analysis and planning are what we do.

We are the Health Planning Council of Northeast Florida We work to improve population health throughout all communities in Northeast Florida by dedicating resources and expertise to health planning and research health promotion and education health data management and analysis. We are the local health council that represents Baker Clay Duval Flagler Nassau St. Johns and Volusia counties. What We Do We bring information to all people in all places where they live work and play We study evidencebased sources to establish facts and reach conclusions We analyze facts and statistics and offer community solutions Why We Do It How We Do It Everything we do is aligned with the conceptual framework developed by the Department and Health and Human Resources HHS and the Centers for Disease Control and Prevention CDC known as the Social Determinants of Health SDOH.
