THE CENTER FOR ADVANCING PUBLIC HEALTH (CENTER) SEEKS TO IMPROVE THE HEALTH OF VULNERABLE POPULATIONS BY ADDRESSING THE FOLLOWING PUBLIC HEALTH FUNCTIONS: MONITOR HEALTH/SURVEILLANCE; INFORM, EDUCATE AND EMPOWER; AND MOBILIZE COMMUNITY PARTNERSHIPS AND LINKAGE TO CARE. THESE THREE AREAS ARE EMBEDDED WITHIN THE CORE FUNCTIONS OF PUBLIC HEALTH AS DOCUMENTED IN THE 1988 INSTITUTE OF MEDICINE REPORT TITLED THE FUTURE OF PUBLIC HEALTH.
