Horizon Housing envisions a world where people and families impacted by disabilities and homelessness are housed in a community of support, where they gain the skills to be self-reliant, and graduate to permanent housing, sustaining themselves and their families. Horizon Housing believes housing is a basic human right. Individuals served by HHDC will experience, compassion, dignity, respect and choice. Each individuals path through Horizon's supportive housing program will be centered on their individual needs, empowered by our person centered, trauma informed, housing coordination of care. (fb)
