Chosen Hearts is a California 501(c)(3) nonprofit organization devoted to supporting individuals and families facing homelessness. Many of those we serve have faced significant challenges, including mental health struggles, substance abuse, domestic violence, and justice system involvement. Our approach goes beyond addressing immediate needs. We engage by outreach and provide direct services and compassionate, personalized case management designed to meet each individual's unique circumstances. Our mission is to rebuild lives and restore hope by fostering a sense of community and creating lasting, positive change. Through our dedicated efforts, we empower those we serve to achieve stability, independence, and a secure, fulfilling future.
