(APPLICANT) IS A NEW YORK NOT-FOR-PROFIT CORPORATION FORMED WITH THE PURPOSE OF BECOMING A DIAGNOSTIC AND TREATMENT CENTER (D&TC) UNDER NEW YORK STATE PUBLIC HEALTH LAW ARTICLE 28 AND APPLYING FOR FEDERALLY QUALIFIED HEALTH CENTER (FQHC) DESIGNATION. FQHCS SERVE MEDICALLY UNDERSERVED AREAS AND POPULATIONS AND PROVIDE PRIMARY CARE SERVICES TO ALL INDIVIDUALS REGARDLESS OF ABILITY TO PAY OR HEALTH INSURANCE STATUS.
