Eligibility and Intake Process
To become eligible for these services, an individual must have a criminal conviction (felony or misdemeanor) and be at least 18 years old. The intake process involves scheduling an appointment with an intake specialist, completing an intake packet, and attending a mandatory orientation session. During this orientation, participants work with a Change Navigator to outline services, set personal goals, and create a needs assessment plan. The Change Navigator follows up with participants for 12 to 16 months to ensure ongoing support.
