(409) 762-2233 The Jesse Tree, a convening organization uses internet technology for community-based Care/Case-management strategies that empower individuals and families to integrate healthcare, social services, faith-based and community resources; connecting them to information and services; providing health education, screening and enrollment; rebuilding their lives and their health through chronic conditions management and uplifting their spirits by breaking the cycles of chronic health and social problems that inhibit self-sufficiency and independence. The Jesse Tree encourages all faiths and denominations to work together as a safety net for those seeking help, assistance and care.
