The care continuum encapsulates all aspects from prevention to end of life care. Hellenic Healthcare Professionals of Michigan offers education about prevention of disease through health screening opportunities lifestyle changes continuing education seminars and various volunteer platforms to educate and identify potentially preventable illness. Wellness Prevention and Lifestyle Education Fellowship HHPM provides an opportunity to form a network of healthcare professionals throughout the care continuum in order to benefit and share services to the communities in which they serve.
