AKRON PHYSICIAN WELLNESS INITIATIVE ("APWI") IS AN OHIO NOT-FOR-PROFIT CORPORATION FORMED ON SEPTEMBER 10, 2020 AND HAS BEEN DETERMINED TO BE TAX-EXEMPT PURSUANT TO INTERNAL REVENUE CODE (IRC) SECTION 501(C)(3). APWI, FORMED BY TWO TAX EXEMPT CHARITABLE HEALTH SYSTEMS (CHILDREN'S HOSPITAL MEDICAL CENTER OF AKRON AND SUMMA HEALTH SYSTEM), TO PRIMARILY PROVIDE SERVICES TO THEIR PHYSICIANS, RESIDENTS, ADVANCED PRACTICE PROVIDERS, AND FELLOWS AFFILIATED WITH THOSE ORGANIZATIONS.
