For those over 55 with chronic complex health conditions or disabilities PACE Southeast Michigan offers a unique lineup of coordinated care to help you stay in the home and community you love. Spend time at our day center filled with caring staff who feel like family and receive medical services like primary care dental vision and physical therapy offered at our facility or in your home. Well even pick you up and drop you off at the end of the day.
