THE CORPORATION WAS FORMED FOR THE PURPOSE OF OPERATING A 36-BED HOME FOR THE AGED LOCATED IN MILAN, TENNESSEE KNOWN AS FOUNTAIN PLACE. THE CORPORATION IS LICENSED BY THE TENNESSEE DEPARTMENT OF HEALTH AND PROVIDES HOUSING, MEALS, SUPPORTIVE SERVICES, AND ASSISTANCE WITH ACTIVITIES OF DAILY LIVING.
